At Swallowfields, we have over 10 years experience in providing personalized care and support to people with learning and physical disabilities, health needs including epilepsy, and Parkinson's disease. We also care for individuals with mental health, including schizophrenia, personality disorder, bipolar disease, and emotional and behavioural
difficulties.
Our dedicated care staff and Therapy team liaises with our local healthcare professionals to ensure that consistent and appropriate input is available. We also have excellent working relationships with community mental health and learning disability teams. We provide highly personalized, safe, and fun environments where residents can develop their social, communication, and life skills with a view to gaining greater levels of independence, including education and employment.

Individual Personalised Care
Swallowfields care centre specialises in supporting adults who require a more structured and meaningful care package. We ensure that the support we provide is personalised based on the individual's needs. Person-centred plans and Health action plans enable us to carry out detailed care and relapse prevention planning whilst measuring progress by working towards a set of reasonable outcomes. These outcomes are identified by everyone we support, which is a fundamental part of achieving greater independence. We provide a highly personalised, safe, and fun environment where service users can develop their social, communication, and life skills with a view to gaining greater levels of independence, including education and employment.
Through our team, we can make huge inroads toward an individual becoming more settled, independent, and involved in the community around him/her.

We play a key part in successfully integrating our service users onto 'step down' services (supported living), therefore minimising the chances of a placement breakdown

We can assist by offering our own supported living facilities within secured tenancies, to ensure that the right level of support is maintained with a consistent approach to staffing.

We can continue to provide long-term support as long as required through our Domiciliary Service.

We can provide support to individuals whose placements have broken down. We can also provide help in cases where there is a 'Safeguarding Alert'
Therapy
At Swallowfields, we employ our own fully qualified therapists who work one to one or in groups with our residents which includes Occupational Therapists, Dance, and Movement Therapists, Art Therapists, Music Therapists, & Relaxation Therapists. We understand that everybody is different, which is why we offer a selection of activities and tailor these to the individual’s choice.
Our professional occupational therapy unit aims to work with our residents in a person-centred way to enable individuals to maximise the following:
Emotional • Cognitive • Personal self-help • Domestic skills • Choice and well-being • Educational skills (such as numeracy, literacy, budgeting, cooking, and healthy living skills) • Community participation including use of public • Calm behavior • Enhance memory • Improve Communication • Psycho-educational • Work on Trauma
We appropriately safeguard people with complex multiple diagnoses whilst supporting them to exercise full choice and control using pro-active and positive means of managing risk. We use clear, multi-agency risk assessments and strategies and high levels of supervision, where required, to facilitate inclusion.
Just For You
We encourage each individual to live their life in a manner that is specific to their wishes, and which promotes independence and choice. We do this by finding out what is important to each resident and supporting them in a way that is meaningful to them. We value the uniqueness of each resident and build on their skills in order to maintain independence and involve them fully in any decision-making processes. We respect and value each resident and their contribution to their home and their care. We ensure that the home provides for all support needs by ongoing assessment and evaluation of each resident ensuring that they are fully involved and consulted at every stage. We establish good channels of communication and information sharing between the next of kin and ourselves and all support given will be transparent, open, and in accordance with the current best practice and legislation.
Learning Disabilities - Home for the less able
Swallowfields Care Centre prides itself on a person-centred approach to the care needs of individuals, promoting independence dignity, and choice. In doing so we aim to create a secure, relaxed, happy, and homely atmosphere for residents and staff. We utilise a dynamic and holistic approach that is directed to the physical, mental, emotional, social, cultural, and spiritual needs.
Our Home
Swallowfields Care Centre is located in Epsom, and is within walking distance to the town centre which has a variety of shops, bars, food outlets, and leisure facilities. The area is well served with public transport links including trains and buses.
Our Facilities
Our home has been specially constructed to support our resident's needs and reflects changing needs, lifestyles and regulations. We are constantly changing and upgrading our environment. We believe that as the needs of our residents change, so should we.
9 Large En-suite Rooms
two of which have wet room facilities, a nurse call system, and TV and telephone points.
2 Lounge Area To Enjoy
A dining room, activities area, gym, conservatory, sensory room, and a safe accessible sensory garden and patio area.
Specially Adapted Vehicles
With a mobility ramp, to accommodate our residents with wheelchairs. Used for daily transport and outings as well as holidays.
2 Kitchens
The second provides therapeutic facilities for our residents to enjoy participating in all aspects of cooking.
Residents have their own individual diaries (some using pictures) and are actively involved in the running of the home, with full support to undertake domestic, gardening, or cooking tasks if they wish. We arrange for entertainers to come into the home; parties, BBQ's and even holidays.
Social Activities
Activities are available in all areas of the home and staff understand the value of providing a stimulating environment and acknowledging that any 'task' can become a meaningful activity with positive support. This may include crafts, leisure, music, spirituality, reading, relaxation, fitness, or visits to local places of interest, shops, and restaurants.
We have our own occupational therapists who provide extensive and appropriate support through detailed cognitive assessments individually tailored to meet the simplest to the toughest personal challenges.
